A day in the life of a Community Matron

First Community’s Community Matrons work to improve the health and quality of life for people with long-term health conditions by coordinating care and services.

They are experienced registered nurses who have advanced skills in areas such as physical assessment, prescribing or have accredited courses in chronic disease management and visit patients in their own homes who are suffering with complex, long-term health conditions such as diabetes, COPD or heart failure.

The Community Matrons review medical issues, long-term conditions that patients may be finding challenging, equipment issues, mobility issues and can also signpost onto other services that they may require. 

They also act as a vital link between other services and professionals to improve the health of people living with long-term conditions and reduce the need for hospital admission. 

Patients, registered with an east Surrey GP, can be referred into the service by their GP or other healthcare professional. Referrals are triaged by the team alongside other healthcare professionals to discuss and agree the care plan for each patient. 

Patients are contacted by the Community Matrons to assess their needs and the team will plan a visit in their own home.
