Mental Health Nurse Practitioners

If you are aged over 18 you may be referred to the service if you have an active physical health need and receiving regular care from another First Community service, and you must continue to be seen by that service in order to receive support.

The team provide general advice relating to mental health and dementia, and initial assessments will take place alongside the First Community healthcare practitioner that made the referral to the Mental Health Practitioners. 

This service is not a mental health crisis service and therefore is not suitable for patients who have active suicidal ideation, present a significant risk of harm to themselves or others, or are acutely unwell requiring psychiatric treatment, such as those experiencing florid psychosis.
